INFORMATION SHEET ABOUT PERSONAL DATA PROCESSING
Dear Customers, we hereby wish to inform you that the data we will come to know when you fill out the contact forms will be processed in accordance with principles of fairness, lawfulness, transparency, and protection of your privacy and rights, in compliance with Legislative Decree no. 196/2003, i.e. Privacy Code on the Protection of Personal Data. The contact forms set out for you on our website have the sole purpose of allowing visitors to contact the website manager, if they so wish, by sending an e-mail message with their contact details and any preliminary notices or questions via the said forms.
This information sheet, released pursuant to Article 13 of Legislative Decree 196/03, concerns the personal data submitted by visitors and users when filling out the contact forms on our website. For this purpose, please note that no other data related to your navigation through our website will be recorded, whilst the data you will voluntarily provide through these modules will be transformed into an e-mail, stored in the e-mail system used by the site owner, and, if deemed appropriate, communicated to the health facilities involved in Bludental project and to the call centre.
1. PURPOSES OF THE PROCESSING OF PERSONAL DATA
Your data may be processed to contact you by the contact details provided by you, and to fulfil any of the requests laid down by you in the message sent to us through the contact forms that can be filled out on our site, to arrange a visit at the most easy-to-reach facility for you, or to send you any reminders regarding appointments/messages or promotions/surveys at the initiative of the facilities adhering to our project.
2. TYPES OF DATA AND METHODS FOR PROCESSING
a) Navigation data: computer systems and software procedures for the operation of this website acquire, during their normal operation, certain personal data whose transmission is implied in the use of Internet communication protocols. These are information that are not collected and saved to be associated with identified individuals, however they are apt, because of their type, to have the users identified, when they are processed and associated with data held by third parties. This type of data includes IP addresses, URI notation addresses of required resources, the time of the request, the method used to submit the request to the server, the size of the file obtained in response, the numeric code indicating the state of the response from the server and other parameters related to the operating system and the computer environment of the user. These data are used for the sole purpose of obtaining anonymous statistical information on the use of the site, and for checking on its proper functioning.
b) Data provided voluntarily by users: if you send an e-mail, at your discretion, expressly, and voluntarily, to the addresses indicated on this website, this will entail the acquisition of your address, necessary to follow up to your request, as well as any other personal information included in the message. The data controller, supported by personnel at each involved unit within Bludental project, will promptly process the data.
c) Cookies: no personal data of users are acquired on purpose by the website. No cookies are used for the transmission of personal information, nor are so-called persistent cookies of any kind used, i.e., systems for tracking users. The use of so-called session cookies (which are not permanently stored on the computer of the user and are deleted when the browser is closed) is strictly limited to the transmission of session identifiers (consisting of random numbers generated by the server), which are needed to allow safe and efficient site browsing. So-called session cookies used on this website avert other information processes apt to impinge on users’ right of privacy when browsing, and do not allow the acquisition of users’ personal identification data.
All personal data are processed by means of automated tools for the time strictly necessary to achieve the purposes for which they were collected. Targeted security measures are taken to prevent data loss, misuse or incorrect use, or unauthorized access.
3. NATURE OF THE PROVISION, AND CONSEQUENCES OF THE REFUSAL
Please note that the provision of your data is optional, aimed only to allow us to contact you and follow up to your request or arrange a visit at one of the facilities adhering to Bludental project. If you refuse to provide and/or consent to the processing of your data for the purposes described herein we will not able to contact you and follow up to your request.

5. DATA SUBJECT’S RIGHTS
At any time you are entitled to query about the existence of your data in our records, obtain the answer, and exercise your legal rights as follows, in accordance with Article 7 of Legislative Decree 196/03:
to receive indications regarding the source, the purposes and methods of the processing; the logic applied to the processing, if the latter is carried out with the help of electronic means; the identification data concerning data controller, appointed data processors and any third parties to whom or which the personal data may be communicated and who or which may get to know said data;
to obtain updating, rectification, integration, erasure, anonymization or blocking of data that have been processed unlawfully, including data whose retention is unnecessary for the purposes for which they have been collected or subsequently processed;
to object, in whole or in part, to the processing of personal data where it is carried out for the purpose of sending advertising materials or direct selling or else for the performance of market or commercial communication surveys.
The abovementioned rights can be exercised by means of requests filed without formalities with the data controller, also through a person in charge, which will be replied to promptly and punctually.
